As a community health provider our mission is to ensure our clients and their families experience quality health care in the comfort of their homes. Our highly qualified and trained staff will ensure communication with your providers ensuring optimal health care at home.
Provides services that consist of non-medical care, supervision, and socialization activities (bowling, movies, etc.) to recipients age 21 years or older. Activities that connect an individual to the community. Care4me will assist and train patients in activities of daily living, such as eating, bathing, dressing, personal hygiene, and preparation of medicines. This service is assigned to encourage community integration and to teach the recipient about home-related responsibilities. Also provides continuous nursing services primarily in licensed residential facility by LPN or RN.
